Often when patients consider getting a hair transplantation operation, the question bothers their minds. Will transplanted hair ever fall out? Well technically, the grafted follicles are immune to hormones causing hair to fall out. But there’s more to this issue than that. First, we have to discuss what causes hair from certain parts of your head fall out while other parts stay healthy.

Each individual produces certain hormones called androgens. Androgens include widely known estrogen and testosterone; they play a critical role in bodily development during puberty and sexual functions. Research shows that a certain cousin of testosterone called DHT (dihydrotestosterone) plays a major role in male pattern baldness. Parts of some people’s hair is sensitive to DHT and follicles around this area start dying in the presence of DHT. Though, as we just said, some parts of our hair, especially parts around the sides and back of our heads are resistant to DHT.

During transplantations, we extract hair from this resistant area to cover up the balding parts. So, the hair follicles themselves do not contain the genetic code that makes them react to DHT. So this explains the technical aspect of transplanted hair not falling out though there are other things to consider in practice.

Sometimes the transplanted hair of some people may seem to fall off. They recognise that after they get more and more open spots. This usually happens to younger patients who have not completely lost their DHT sensitive hair. Though, the transplanted hair stays where it is. The lost hair around it causes it to seem like it’s falling.

Doctors often warn younger patients seeking hair transplantations that they may need another operation in the future due to this. Some patients go for early intervention. They see the first signs of hair loss such as thinning crown hair and regressing forehead line. Even after getting an operation to fill up space, they are still likely to see some loss of coverance.

The science still cannot offer us a solution to keep our DHT sensitive hair without the risk of side effects. Finasteride and minoxidil offer management therapies with a relatively low side effect profile. But they are just management therapies. In other words, as soon as the patient ceases treatment, the follicles genetically programmed to die to start dying once more.
